End of Season Report Card

I have coached a 5th & 6th grade team for the past 3 years and also coached the 7th & 8th grade team as well this year. Me and the other coach are going to build a system or program at the school over the next 4 years at least.
My thought is to give the boys a report card with what they got better on during the season and what they still need to work on. Now I know everyone can always work on all parts of the game. But I want my point guard working on attacking with his weak hand and my low post player working on post moves.
I am wondering if anyone might have a template they use for something like this.

I give players a rough evaluation. Then give notes below on specific recommendations.

I'll paste our evaluation form here but the table formatting is lost. This is normally formatted in a cleaner looking table.


* Scale of 1 to 5 (with 5 being highest and 1 being lowest).
Defense (on ball, help, pressure, dictating) 3 / 5
Dribbling 2 / 5
Passing - accuracy and vision 2.5 / 5
Ball Security 2 / 5
Rebounding 4.5 / 5
Team Offense (execution of offense, passing, creating shots for teammates) ? / 5
Scoring 2 / 5
Court sense & decision making 4 / 5
Aggressiveness 4.5 / 5
Leadership (communication, chemistry, making teammates better, etc) 3 / 5
