Transition Offense and the Four-Second Fast Break

Great transition system that I plan to use for my high school team next year. What adjustments would you make when defense attempts to stop outlet pass to point guard? Would you go to opposite wing who would then replace the guard position on the break or wait for the guard to get open and pass him the ball?

This may sound a little trite, but my point guard is always open.
This is a function of two things, practice and mentality. Since we practice it every day, we practice what is necessary to get the ball to the PG. The second thing is the PG has to have the mentality that he will get the ball, regardless of how they try to defend him. We do this every day. My message is, "If you can't get the ball, you can't play!"
Then, in the real world, yes. When you look at the way the break is run, all the positions are essentially interchangeable. As, long as the ball gets up the court for a layup or to the corner, it doesn't matter who gets it there. If you inbound to a wing, the PG runs the lane and we try to headman pass to him as he runs to the corner. If the ball handler (whomever he might be) is ahead of the PG, the ball handler will take the ball to the corner and the PG will fill the top spot.
