Parent's Release and Indemnity Agreement to Breakthrough Basketball LLC Basketball Training

In consideration of the acceptance of this application for enrollment in any of the Breakthrough Basketball LLC Basketball Training, the undersigned participant and parent / legal guardian of participant (if under 18 years of age), intending to be legally bound, hereby for myself/ourselves, my/our heirs, executors and administrators, waive and release any and all rights and claims for injuries and damages I/we may have against Breakthrough Basketball LLC Basketball Training, all sponsors and all employees and agents of Breakthrough Basketball LLC Basketball Training for any or all injuries and damages which may be sustained and suffered by the participant in connection with my/our association with, or entry into this training, and which may arise out of my traveling to, participating in or returning from the training.

I/We hereby grant permission for my/our child to be a participant in the specified Breakthrough Basketball LLC Basketball Training and if an injury should occur during, traveling to or returning from the training, I/we agree to pay for all costs, present and future, through my/our medical insurance policy and/or personal finances.
